Home Health Aide / Community Health Representative (CHR)

Overview:
Provides home‑based supports, outreach, system navigation, and culturally grounded health promotion. HHAs focus on personal care and daily living supports; CHRs serve as liaisons between providers and community members to support access and education.

What You’ll Do:

  • Home visits, vitals, and basic supports within scope.
  • Accompany clients to appointments; navigation and advocacy.
  • Health promotion, materials, and reminders.
  • Community outreach and program support.

Prerequisites:

  • Grade 12 diploma — Regular Grade 12, Adult 12, or GED.

Education & Qualifications:

  • CHR/HHA training; knowledge of community strengths and services.
  • Relationship‑building, reliability, cultural safety.

Where You’ll Work:

  • Client homes, community settings, clinics.

Growth Pathways:

  • Senior CHR → Coordinator → Nursing/Allied Pathways (with training)
