Overview
Provides broad nursing services across the lifespan, including immunization, chronic disease support, assessments, home visits, outreach, and community‑based education.
What You’ll Do
- Conduct assessments, care planning, and referrals.
- Support immunization and chronic disease programs.
- Deliver home visits and community outreach.
- Provide health education and advocacy.
Prerequisites
- Grade 12 diploma (Regular, Adult 12, or GED).
- Must enter RN or LPN pathways.
