Overview

Provides broad nursing services across the lifespan, including immunization, chronic disease support, assessments, home visits, outreach, and community‑based education.

What You’ll Do

  • Conduct assessments, care planning, and referrals.
  • Support immunization and chronic disease programs.
  • Deliver home visits and community outreach.
  • Provide health education and advocacy.

Prerequisites

  • Grade 12 diploma (Regular, Adult 12, or GED).
  • Must enter RN or LPN pathways.
