Overview:
Provides home‑based supports, outreach, system navigation, and culturally grounded health promotion. HHAs focus on personal care and daily living supports; CHRs serve as liaisons between providers and community members to support access and education.
What You’ll Do:
- Home visits, vitals, and basic supports within scope.
- Accompany clients to appointments; navigation and advocacy.
- Health promotion, materials, and reminders.
- Community outreach and program support.
Prerequisites:
- Grade 12 diploma — Regular Grade 12, Adult 12, or GED.

- Practical Nursing, Indigenous Practical Nursing

Education & Qualifications:
- CHR/HHA training; knowledge of community strengths and services.
- Relationship‑building, reliability, cultural safety.
